All you need to do is go to
www.myshps.com and login to your account. It shows your annual elections and the amounts that have been used for all your FSA accounts. All the forms are online and can be printed from pdf once you login.
OR you can call SHPS at 866-362-6993.
Pre Tax = gotta love it. :-)
